What we need to accept a referral, what we send back, and how we handle complex or urgent starts.

What we need from you

A complete referral means we can give you a yes or no within two business days, rather than three rounds of email.

  • Participant name, contact and plan management type
  • Plan dates and the relevant support categories with available funding
  • The supports and indicative hours sought, and preferred days
  • Any behaviour support plan, mealtime management plan or nursing care plan already in place
  • Risk information we need before a first shift — access, pets, other household members, known triggers

What you get back

  • A written capacity response, including honest notice if we cannot staff it well
  • A draft service agreement and schedule of supports for your review
  • Progress notes and reporting at the frequency you specify
  • A named coordinator as your single point of contact — not a shared inbox

Complex and urgent referrals

For high-intensity or behaviour support referrals we complete a clinical assessment and train workers against the participant-specific protocol before the first shift, so the start date is realistic rather than optimistic. If a placement is at risk of breaking down, call the office directly on 02 9067 1097 and we will tell you the same day whether we can help.
